At Taproot Community Support Services, our Family Resource Centres (FRCs) provide inclusive, accessible support to help families and individuals navigate disability services, access resources, and work toward greater independence. Whether seeking guidance for FSCD or PDD services or needing personalized support, our team is here to help.

How It Works

We take a structured, person-centred approach to ensure individuals receive the right support at the right time:

Referral

  • We accept referrals from Disability Services and self-referrals.
  • If self-referring, we help families confirm eligibility for PDD or FSCD services and guide them through the intake process.

Intake & Needs Assessment

  • We meet with families to understand their goals, explain available supports, and outline their rights and responsibilities.

Personalized Support & Training

  • Based on the assessment, we provide tailored services to meet each person’s needs.
  • Regular check-ins ensure progress, and services are adjusted based on feedback.

Ongoing Review & Support

  • We continuously monitor progress and make changes to help individuals reach their goals.

Completion & Transition

  • Services conclude when goals are met, a person chooses to withdraw, or support is no longer needed.

Self-Referral Process

Families can now connect directly with FRCs through a streamlined self-referral process:

  1. Eligibility Check – We confirm eligibility for PDD or FSCD services.
  2. Service Fit – We ensure the requested support aligns with our services.
  3. Verification – Once confirmed, we complete the FRC Self-Referral Verification Form and begin support.
